ABOUT THE JOB
As part of the Care Coordination Team the Case Manager performs work to provide outreach, linkage, care coordination and retention services to individuals experiencing homelessness. This position is responsible for providing field based, direct client services alongside a team of housing locators, and community providers.
ESSENTIAL DUTIES:
Conduct client intake and assessment for the program to identify clients’ individualized needs for services.
Collaborate with clients to develop individualized treatment plans that address short- and long-term goals.
Provide strengths-based case management services and service coordination designed to assist clients in obtaining and maintaining stable housing and employment.
Provide on-going case management support to assess client progress and ensure treatment plan outcomes are met or changed as necessary.
Conduct crisis and risk assessments in consultation with supervisor/ team or LCSW.
PREFERRED QUALIFICATIONS
Two (2) years’ experience working in a relevant field.
HMIS Experience.
Demonstrated knowledge of and/or experience with advanced and evidence-based case management interventions such as, but not limited to:
Harm Reduction strategies
Motivational Interviewing
Housing First Practices
Non-violent Crisis Intervention
Critical Time Intervention
Other case management techniques proven effective with vulnerable populations.
MINIMUM QUALIFICATIONS

DRIVING Driving is an essential function of this position. To meet the employment requirements for this role, all candidates must:
Have reliable transportation
A valid driver’s license
Proof of insurance and ownership for personal vehicles used during work duties
The ability to qualify for PATH's insurance coverage
