JOB SUMMARY:

Orange County PACE is a comprehensive healthcare and wellness program designed to help seniors live independently at home as an alternative to nursing care. Their network of dedicated healthcare professionals provides individualized, coordinated care from medical services to home medication delivery, transportation to nutritious meals. Their PACE team approach to care utilizes a unique, world-class delivery system designed to eliminate disparities in healthcare access for underserved populations, leading to successful outcomes for diverse communities of seniors and their families.


RESPONSIBILITIES:

  • Obtain Hands-on experience in healthcare and gerontology.

  • Understand the importance of culturally-appropriate health education for older adults and their families.

  • Examine the most common health problems unique to the older adult population.

  • Understand the philosophy at PACE is to help participants remain as independent as possible, living in their own community and home.

  • Understand the process of case management from the time they enrolled and through their journey and participation in the program.

  • Understand how Inter Disciplinary Team members (IDT) function and make the decisions for participants overall well-being.

  • Understand that our health care professionals assess and evaluate changes, provide timely intervention and encourage participants to help themselves.

  • Network and assist various healthcare professionals in a rotational capacity.

  • Give examples of community resources through which older adults and their families can be referred for support.

  • Ability to work with confidential information.

  • Ability to work effectively with all levels of management.

  • Familiarity with issues and needs of the community.


SKILLS DEVELOPED OVER WORK EXPERIENCE:

  • Learn to provide culturally appropriate health education for older adults and their families.

  • Understand the process of case management from the time they enrolled and through their journey and participation in the program.

  • Learn about community resources through which older adults and their families can be referred to for support. Learn to refer clients to resources appropriately.

  • Network and assist various healthcare professionals in a rotational capacity.

  • Understand how Inter-Disciplinary Team members (IDT) function and make the decisions for participants overall well-being.

  • How to maintain confidential information.


QUALIFICATIONS:

  • Proficient knowledge of computer skills. MS Office (Word, Excel, Access, PowerPoint, and Outlook) preferred.

  • A solid foundation for pursuing careers in healthcare, gerontology, nursing, medicine, social work, or related fields.

  • Ability to communicate effectively, both orally and in writing.

  • Preferred skills:

-Attention to detail and accuracy.

-Good personal organization skills, ability to handle multiple tasks and prioritize

and use time effectively.

-Patience, willingness to learn, and open mindedness to differing perspectives

and ideas.

-Commitment to the mission of the project and its impact on schools and

communities.

-Ability to work effectively with diverse populations.
