Position Summary:

Seeking a dedicated and compassionate Care Manager/Coordinator to join the team. This role involves conducting orientations, assessments, and supervisory visits for the home care field staff and Home Care Aides. The Care Manager/Coordinator will ensure the quality of care provided to the clients, maintain up-to-date care plans, and support caregivers to foster a safe, effective, and compassionate environment. This role is flexible in scheduling, allowing you to make a difference while balancing other commitments.


Essential Job Duties:

  • Caregiver Orientations and First Day Support: Accompany newly hired caregivers on their first assignment, providing support, guidance, and an introduction to the care plan to ensure safety and client satisfaction.

  • Client Assessments and Care Plans: Conduct initial assessments for both private pay and referred clients, developing personalized care plans tailored to each client's needs.

  • Quarterly and Routine Supervisions: Conduct supervisory visits every 90 days for all caregivers, assessing their ability to follow care plans, providing instruction, and gathering client feedback to ensure the highest quality of care.

  • Emergency Supervisions and Retraining: Respond to client complaints or caregiver challenges by providing emergency supervision visits or retraining, as needed, to ensure that care standards are maintained.

  • Client and Caregiver Communication: Engage regularly with clients, their family members, and caregivers to address concerns, answer questions, and update care plans based on changing client needs.

  • Human Resources Functions: Participate in interviewing, reference checks, and ongoing training to ensure the best talent joins our team and continues to develop.

  • Supportive Team Environment: Collaborate with a dedicated team committed to providing high-quality care.

  • Growth Opportunities: Participate in ongoing training and education to further develop your skills in care coordination.


Requirements/Qualifications:

  • College education preferred; relevant healthcare experience will be considered.

  • Some experience in home care or community healthcare setting.

  • Strong interpersonal skills with the ability to communicate effectively with clients, caregivers, and office staff.

  • Some familiarity with caregiving practices, including use of Hoyer lift and oxygen tanks preferred.

  • Ability to manage time efficiently, work independently, and provide supportive leadership to the caregiving team.

  • Computer literacy, including comfort with basic software for record-keeping and communication.
